OakLeaf Clinics is dedicated to providing our patients with compassion, trust, and a lifetime of individualized care. Our healthcare team consists of physicians, nurse practitioners, physician assistants, dietitians, nurses, respiratory therapists, and medical assistants working in concert with laboratory/imaging services to offer individualized care to the Chippewa Valley.

Position description

The registered nurse collaborates with the outpatient treatment team to coordinate care for our patients, including triaging patient needs over the phone and assisting the clinical team with in-person patient care (e.g., IV fluid administration, suture removals, medication administration (IM/SQ)).

Responsibilities

• Deliver compassionate, patient-centered care in an outpatient setting
• Create, coordinate, and implement the patient plan of care in collaboration with the healthcare team
• Use the nursing process to provide ongoing care (assess, plan, intervene, and evaluate)
• Recognize and respond to changes in the patient's status and need for care
• Determine priorities of patient care based on essential patient needs
• Provide triage and consultation to patients, family, and/or significant others
• Communicate with physicians and other members of the health care team and document accurate and timely information regarding patient conditions
• Follow through with necessary procedures, test requests, prescription refills, new appointment times, and referrals to other facilities or services
• Refer questions to the physician/provider and relay information back to the patient as directed
• Perform advanced clinical tasks or skills, such as the removal of sutures
• Maintain flexibility in scheduling to meet clinic needs

Qualifications

• Graduate of an accredited school of nursing
• Current State of Wisconsin Registered Nurse License or Nurse Compact License
• Basic Life Support (BLS) certification, Advanced Cardiac Life Support (ACLS) awarded by the American Heart Association (AHA) within three months of hire
• Previous RN experience required (outpatient preferred)
• Excellent verbal and written communication skills 
• Experience working in a team environment
• Ability to adapt to unpredictable situations within the work setting
• Demonstrated leadership and excellent critical thinking skills
• Epic EMR experience (preferred)

While performing the duties of this job, the employee regularly works in a clinic or office setting.

Physical demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Major activities of this job include sitting frequently for extended periods, some walking or standing, and light physical effort performed on a level surface. Some carrying, lifting, kneeling, bending, reaching, and squatting are involved in the medical office. Use a computer and telephone and employ writing and organizational skills. Manual dexterity, mobility, and good vision, including near acuity, depth perception, and accommodation, are physical requirements for technical work.

Salary: Starting at $32.50, commensurate with experience
